As an employee or retiree, the health benefits available to you represent a significant component of your compensation package. They also provide important protection for you and your family in the case of illness or injury.
Depending on your district, you may be offered a series of health coverage options. Choosing a health coverage option is an important decision. To help you make an informed choice, SWSCHP makes available a Summary of Benefit Coverage (see the two links below), which summarizes important information about any health coverage option in a standard format to help you compare options.
A paper copy is available to you upon request to your District’s Health Benefit Representative.
Forms and Documents
To Our SWSCHP Active Members – the SPD (Summary Plan Document) defines what is covered by the plan and what you (personally) must pay for. Each year, a revised SPD will be posted to the website for SWSCHP members to use as a ready reference. It is a ‘living’ document that is constantly evolving based upon regulations, new developments or practices in healthcare, and the specific plans that we negotiate for on behalf of our members. As such, members might notice changes on a year-over-year basis to the SPD. This SPD replaces the Summary of Benefit Coverage (SBC) for BCBS members that was posted earlier in the year . Please note that while it has not yet been formally approved by the NYS Department of Financial Services, SWSCHP is fully honoring all aspects of this SPD.
