From this website you can update your Primary Care Physician (PCP), find details about your claims, see how your claims have affected your plan Deductible and Out-of-Pocket Maximum, Connect with LiveHealth Online, and special offers and discounts. Click here to see the Summary Plan Document (SPD) and the Summary of Benefit Coverage (SBC).
Create or login to an existing Anthem account.
Claims Administration regarding Anthem will take place through Amalgamated (AEBA), our third party benefits administrator. A separate AEBA account must be established by members. You can log in to your account to view your claims, order a replacement ID Card and create a user account.
